The blue Check Out link will take you to an online form that allows you to enter your credit card information, and the amount you wish to pay. A 3% convenience charge will be added to each transaction.
Please include your Patient ID – from your invoice – type it into the “Order ID” field. The PO field is optional. Thank you!
